In order to request a tutor, please fill out the following form and read carefully the conditions below.

By submitting this form, the student-athlete agrees to the following:

1.

Be on time for all tutoring appointments. Tutoring sessions will last one hour and you must meet at least once a week.

2.

Attend all classes and labs. If you miss class because of a game, find a classmate to get notes. Tutors are not responsible to teach material you missed because of a game.

3.

Bring course syllabus, book and notebook.

4.

Develop a plan of action with tutor. Meet with your professor to discuss effective study strategies.

5.

Try to do homework/reading or assignment before meeting with the tutor. Write down questions or problems.

You must call or e-mail your adviser a day in advance to change or cancel an appointment. Three combined "no shows" for tutoring or individual meetings with a student-athlete adviser will result in a loss of tutorial services for the remainder of the semester. Also, your tutoring is a weekly appointment.

A copy of this agreement will be sent to your coach.
